Note: Depression should not be diagnosed or excluded solely on the basis of a PHQ-9 score. A PHQ-9 score ≥ 10 has a sensitivityof 88% and a specificity of 88% for major depression.1 Since the questionnaire relies on patient self-report, the practitioner shouldverify all responses. A definitive diagnosis is made taking into account how well the patient understood the questionnaire, as wellas other relevant information from the patient.
PHQ 9
The PHQ-2 inquires about the frequency of depressed mood and anhedonia over the past two weeks. The PHQ-2 includes the first two items of the PHQ-9. The purpose of the PHQ-2 is to screen for depression in a “first-step” approach.

Generalized anxiety disorder (GAD) is one of the most common mental disorders; however, there is no brief clinical measure for assessing GAD. The objective of this study was to develop a brief self-report scale to identify probable cases of GAD and evaluate its reliability and validity.
GAD7
The first diagnostic criterion for PTSD is having experienced a traumatic event, which DSM-5 defines as “exposure to actual or threatened death, serious injury, or sexual violence” (APA, 2013). The person must experience symptoms across four clusters: intrusions, avoidance, negative alterations in mood or cognition, and alterations in arousal. Symptoms must be present for more than one month and result in significant distress or impairment
PCL-5
The PCL-5 is intended to assess patient symptoms in the past month. Versions of the PCL-5 that assess symptoms over a different timeframe (e.g., past day, past week, past 3 months) have not been validated. For various reasons it often makes sense to administer the PCL-5 more or less frequently than once a month, and in those cases the timeframe in the directions may be changed to meet the purpose of the assessment, though providers should be aware that such changes may alter the psychometric properties of the measure.
